The child death review database project is designed to specify the key elements required to develop a ‘national' database for England and Scotland to collect information from the Child Death Reviews which are carried out in England by Child Death Overview Panels (CDOPs) and from the Child Death Review process which is currently being developed in Scotland.
- Child Head Injury Project

Traumatic head injury is amongst the most common cause of morbidity, mortality, disability and lost years of productive life in children. In 2009, The Centre for Maternal and Child Enquiries (CMACE) collected data on approximately 6,000 children who attended hospital with head injury in England, Wales, Northern Island, the Isle of Man and the Channel Islands. This was part of The Confidential Enquiry into Head Injury in Childhood.

Part of the National Cardiac Audit Programme (NCAP), the purpose of the audit is to stimulate quality improvement through the provision of comparative information on: the structure and activity of PCI services; the access to, appropriateness and quality of care against national standards; outcome for patients such as complications, adverse cardiac events and death/survival.

The Myocardial Ischaemia National Audit Project (MINAP) was established in 1999 in response to the national service framework (NSF) for coronary heart disease, to examine the quality of management of heart attacks (myocardial infarction) in hospitals in England and Wales. Part of the National Cardiac Audit Programme (NCAP), the audit aims to improve the quality of care and outcomes of patients who have heart attacks. It aims to improve the whole pathway from the call to the emergency services, to the prescription of preventive medications on discharge from hospital. The audit describes aspects (process measures) of the quality of care of hospitals and of ambulance trusts and is based on analyses of data that have been directly submitted by the participating organisations.

Part of the National Cardiac Audit Programme (NCAP), the audit aims to reduce mortality and improve outcomes for patients undergoing adult cardiac surgery. It reports comparative data on all major heart operations carried out on NHS patients in the UK.

Congenital heart disease refers to any defect of the heart present from birth. It includes structural defects, congenital arrhythmias (heart beats with an irregular or abnormal rhythm) and cardiomyopathies (abnormal heart muscles). At least eight in every 1,000 babies are born with a heart or circulatory malformation, although only a quarter to a third of these are detected by scans (antenatal ultrasound). Diagnosis and treatment of complex heart defects has greatly improved over the past few decades and almost all children who have heart malformations survive to adulthood.

Heart failure results in a large burden on the NHS, accounting for around one million inpatient bed-days – 2% of the NHS total – and 5% of all emergency hospital admissions. Part of the National Cardiac Audit Programme (NCAP), the audit aims to improve the quality and outcomes of care for patients with unscheduled admission to hospital with heart failure. It captures data on clinical indicators which have a proven link to improved outcomes and encourages the increased use of clinically recommended diagnostic tools, disease-modifying treatments and referral pathways.

Part of the National Cardiac Audit Programme (NCAP), the audit details clinical activity in the fields of: pacemakers; implantable defibrillators; and cardiac resynchronisation therapy. Implant rates and recent trends in these rates are presented for the UK as a whole (alongside other European nations), for the constituent nations in the UK, and for each Clinical Commissioning Group in England.

Heavy menstrual bleeding (HMB) is a prevalent condition that affects 20 - 30% of women of a reproductive age. The Heavy Menstrual Bleeding Audit was commissioned in England and Wales between 2010 – 2015 as part of the centrally-funded National Clinical Audit and Patient Outcomes Programme (NCAPOP). It was managed by the Royal College of Obstetrics and Gynaecology (RCOG).

- Learning disability mortality review programme

Programme overview The Learning Disability Mortality Review Programme (also known as LeDeR) was established to drive improvement in the quality of health and social care service delivery for people with learning disabilities (LD) by looking at why people with learning disabilities typically die much earlier than average. The programme was commissioned by HQIP on behalf of NHS England and was led by the University of Bristol’s Norah Fry Research Centre. Through local retrospective reviews of deaths, health and social care professionals, and policymakers have been supported to understand causes of death and to identify amenable factors contributing to the overall burden of excess premature mortality for people with learning disabilities. The programme has identified variation and best practice and made several recommendations to improve services.
- National Audit of Breast Cancer in Older Patients (NABCOP)

Programme overview NABCOP was a national clinical audit which began in April 2016 and was run by the Association of Breast Surgery (ABS) and the Clinical Effectiveness Unit (CEU) of the Royal College of Surgeons of England (RCS). The aim of NABCOP was to support NHS providers to improve the quality of hospital care for older patients with breast cancer. This was achieved by publishing information about the care provided by all NHS hospitals that deliver breast cancer care in England and Wales, looking at the care received by patients with breast cancer and their outcomes.

The National Clinical Audit of Anxiety and Depression (NCAAD) programme closed on 31 July 2020. NHS England and NHS Improvement (NHSE/I) decided not to extend the NCAAD contract for this audit, on the grounds it was not possible to adjust/alter the current audit sufficiently to underpin key components of the NHS Long Term Plan.

Programme overview The National Clinical Audit of Specialist Rehabilitation for Patients with Complex Needs Following Major Injury was a three year programme conducted in partnership with the British Society of Rehabilitation Medicine (BSRM) and led by the UK Rehabilitation Outcomes Collaborative (UKROC), London North West Healthcare NHS Trust, the Trauma Audit and Research Network (TARN), Manchester University, and the Cicely Saunders Institute, King’s College London. It provides a comparative assessment of services provided in this area in relation to specialist injuries caused by events such as road accidents and falls.
- National Head and Neck Cancer Audit

The head and neck cancer audit was commissioned in England and Wales from 2005 to 2017 as a topic in the centrally-funded National Clinical Audit and Patient Outcomes Programme (NCAPOP). The most common sites for head and neck cancer are the larynx (throat) and oral cavity (mouth). Head and neck cancer treatment requires a wide range of expertise, and treatment is usually discussed and agreed by multidisciplinary teams (MDTs). The aim of this audit was to produce meaningful results that act as a vehicle to improve delivery of care to patients.

Mastectomy (removal of all breast tissue) is a treatment undergone by many women with breast cancer. The breast mound may be reconstructed at the time of mastectomy (immediate reconstruction) or at a later date (delayed reconstruction).

The National Mortality Case Record Review (NMCRR) programme aims to develop and implement a standardised way of reviewing the case records of adults who have died in acute hospitals across England and Scotland by improving understanding and learning about problems and processes in healthcare associated with mortality, and also to share best practice. The NMCRR programme is a national collaborative project led by the Royal College of Physicians (RCP) in partnership with Yorkshire and Humber Academic Health Science Network’s Improvement Academy and Datix.

National Ophthalmology Database (NOD) Audit Programme The Royal College of Ophthalmologists (RCOphth) was commissioned from 2014 to 2019 by the Healthcare Quality Improvement Partnership (HQIP) to run the National Ophthalmology Database (NOD) Audit Programme as part of the National Clinical Audit and Patient Outcomes Programme (NCAPOP). The NOD audit collects data on cataract surgery performed in England and Wales and provides individual surgeons, healthcare providers and the public with benchmarked reports on performance, with the aim of improving the care provided to patients.

The National Pain Audit was commissioned in England and Wales between 2009 – 2013 as a result of the Chief Medical Officer's report of 2008: ‘Pain: Breaking Through the Barrier' which expressed concern that the needs of people with chronic pain were being poorly served by the NHS and wider society. It formed part of the centrally-funded National Clinical Audit and Patient Outcomes Programme (NCAPOP) and was managed by the British Pain Society and Dr Foster Intelligence.
