Confidentiality agreement samples

These confidentiality agreement samples were provided by Northumberland Tyne and Wear Foundation Trust and NHS Petersborough Community Services, in 2010 and 2011 respectively, as a result of working with patient panels and user groups as part of clinical audit and quality improvement projects. It is important to have an agreement in place if volunteers are finding out information about an organisation’s patients and service users.

These agreements are referenced in the following HQIP guide, A guide to developing a patient panel (revised 2016).

Clinical audit manual for lay members of audit teams

This training manual was developed by HQIP Service User Network member Margaret Hughes in 2012 having trained lay members of Wrightington, Wigan and Leigh NHS Foundation Trust’s patient panel in clinical audit. The manual covers the history, objectives, structure and planning techniques behind effective auditing and comes highly recommended by staff and volunteers alike at the Trust. It will be referenced in Developing a patient and public involvement panel for quality improvement (2016) which will be made available soon.
