Excellence in clinical audit and quality improvement techniques, helping to fuel real and impactful improvements in health and care, must be demonstrated.

Please also make clear how the following points have been met, in your nomination:

• Must involve clinical audit(s)* or similar evidence/data-informed healthcare improvement project(s)
• The project period must be as recent as practically possible (ideally run since October 2022; however, we recognise – and accept – that some projects may involve longer lead times)
• Those nominated must have led or made a substantial contribution to the successful delivery of the project
• Shows evidence of sustainable impact, whether environmentally, organisationally or any other way.

• Individuals or teams can be nominated by themselves or by others
• Projects, individuals or teams can be involved in any type of healthcare setting in the UK
• Open to projects at a local, system or national level.

• Clear need identified
• Championing data-driven and evidence-based improvement as good practice
• Clear project design with a focus on sustainability
• Evidence of engagement with stakeholders
• Successful approach.
